You can see the full in-game text for the collaboration between Fortnite and Disney Plus just below, which was unearthed through the latest patch to hit Epic's game. According to the datamine, any purchase in Fortnite's Item Shop until December 31 will reward you with two free months of Disney Plus.

Any Fortnite In-Game Purchase Gives Players Two Months of Disney Plus
To claim the subscription, players need to go to the this page on the Fortnite website on Nov. 10 and follow the steps described there. Only one offer code can be applied to each Fortnite account. The offer is open to new Disney+ subscribers 18 years old and older and living in any of the 21 countries listed here, including the United States, United Kingdom, Canada, and Australia. Once the free subscription period expires, players will be automatically charged the standard monthly rate unless the players cancel beforehand.
